Before you Begin
Federal forms must be generated prior to completing the steps below. If you have not generated the federal forms yet, please refer to How do I generate and review Federal 1094 and 1095 Federal forms?, then How do I Print, Distribute and Submit Federal 1094 and 1095 forms?
The majority of states do not need to file State 1094 and 1095 forms. Only California, New Jersey, Rhode Island, and Washington DC need to submit state 1094 and 1095 forms.
Determine your State deadline
Some states have different deadlines for their forms. The deadlines are listed below for these states.
- California: Deadline for 1095 to employees is January 31 and the filing deadline is March 31st
- New Jersey: filing deadline March 31
- Rhode Island filing deadline March 31
- Washington DC filing deadline April 30
State Filing Setup
Once you know your state deadlines, you need to set up your state filing. Follow the steps below to complete the state filing setup. This will be the same process for California, New Jersey, Rhode Island, Washington DC and Self-insured Massachusetts.
- From the ACA main menu, select ACA, then State Filing.
- Select Employee State Filing Setup.
- Check in the upper corner that you are in the Setup screen. The Setup tab should be red.
- If needed from the Employer field, select the location.
Note: Most users complete the setup for all locations. - From the Employee Address State, select the state you are preparing to file.
- From the Federal Forms Exists field, select Yes.
- From the table at the bottom of the page, select the top checkbox to select all employees for the desired state, then go to Generating 1095 Forms below.
- Select Save to keep your changes.
- Go to Generating 1095 Forms.
Generating 1095 Forms
Once the setup is complete, the 1095 forms can be generated. Follow the steps below to complete this process.
- From the upper left corner, select the 1095 tab.
- From the Reporting State field, select the appropriate state.
- Set additional fields as needed.
- From the middle of the page, select Generate Forms for Organization.
- Go to Generating 1094 State Forms.
Note: State forms do not need to be printed.
Generating 1094 State Forms
- From the upper left corner, select the 1094 tab.
- From the Reporting State field, select the appropriate state.
- Set any additional field filters as needed.
Select Generate Forms for Organization.
Note: States do not need their forms printed. - The completion of the final steps vary depending on your state. Select your state from the list below to complete the generation of the state forms.
CALIFORNIA and NEW JERSEY STATES ONLY
- After the forms have been generated, select, Submit Forms for Organization to File.
- This procedure is complete.
WASHINGTON DC
- After the forms have been generated, select, Submit Forms for Organization to File.
Note: Only Vendors can upload these files. - Notify Points North that the state 1094 and 1095 forms have been submitted.
- This procedure is complete.
RHODE ISLAND ONLY
Rhode Island needs to download a file for each location, then upload them to the Rhode Island system. Follow the steps below to complete the state filing for Rhode Island.
- After the forms have been generated, select, Submit Forms for Organization to File.
- From the top menu, select ACA, then Transmissions.
- The Transmissions page appears containing a file (for one location) or a list multiple files (for multiple locations).
- For Users with a single location, select Download CVS to the right of the file.
- For Users with multiple locations, select Download All Files at the bottom of the table. The Download All Files will create a .Zip file that contains a copy of each .csv for every location under the organization. This process will take longer than generating individual files.
- Navigate to the State of Rhode Island Division of Taxation page: Individual Mandate Reporting (ri.gov).
- Follow the instructions to upload your file. When prompted, navigate to your local download folder, then select the .zip file (for multiple locations) or .cvs file (for single locations) to upload your file to the State of Rhode Island Division of Taxation.
- This procedure is complete.
SELF-INSURED MASSACHUSETTS ONLY
ACA Reporter does not support the filing requirement for the state of Massachusetts.
